Assessment of your cardiovascular risk
Address each cardiovascular risk factor (ex. High blood pressure, diabetes or pre-diabetes, cholesterol)
Provide a detailed plan with you for managing each of these conditions
Online group sessions to discuss each risk factor, nutrition, physical activity, and more
What to expect
Your First Visit
Please bring the following items to your first visit:
1. Government issued ID
2. List of current medications
3.   Recent blood pressure readings at home (if possible)
What to expect
1. You will have your blood pressure, height, and weight assessed
2. You will then meet with the doctor who will review your medical history, and discuss your goals for your health
3.   Together you will make a plan to improve your health plan
Follow-Up Visits
Please bring the following items to your follow-up visits:
1. Government issued ID
2. List of current medications
3.   Recent blood pressure readings at home (if possible)
What to expect
1. You will have your blood pressure assessed
2. You will then meet with the same doctor to review recent tests and progress on your medical conditions
3. Together, you will make a plan to continue improving your health
Eligibility and Referrals
Referrals must be sent by family physicians or specialists. Patient self-referral is 
not accepted.
